Attestation

The person who completed this page has confirmed that all required studies and consultations have been performed, and that the application has been reviewed and certified by a qualified
professional. One Attestation page must be completed per licence application submission. While
the on-line application system is based on the "honour system" (that is, it is assumed that the
attestation and certifications being made are correct), the Department will conduct random
checks of applications to ensure that the system is not being abused in any way.

Delete Attestation Data
Click to delete the attestation from the application. Note that this function is only available for applications that have not been submitted.
Attestation Details
Shows the status of studies and consultation, and conformity to regulations and standards.
