Re: Voluntary Refit Model MD3021 AirForce Inflatable Pouch PFD

The MD3021 inflatable pouch is in compliance with UL1180 requirements, the Canadian Addendum, and Transport Canada approval, however we have determined that the potential exists for a possible separation of the inflatable cell from its belted pouch carrier during inflation.

Mustang Survival is requesting all MD3021 inflatable pouches be returned to our factory for refit. Check the diagrams below to see if your unit needs to be refit. You will be responsible for shipping your product back to us. It will be returned to you, as quickly as possible, pre-paid. You will need to obtain a return authorization number from our Customer Service Department and we request that you ship your unit without the CO2 cylinder. Please send your unit back to Mustang Survival, Unit #104, 3820 Jacombs Road, Richmond BC V6V 1Y6. For a return authorization number or for further information please feel free to call our Customer Service Department at 1.800.661.6181 or email us at mustang@mustangsurvival.com.

Original and Refitted Designs

Thank you very much for your cooperation with this important matter. Yours Sincerely,
