Where can I find a generic transportation of dangerous goods form?

There is no form required to transport dangerous goods and Transport Canada does not issue any forms to be used for this purpose. To legally transport dangerous goods, a document only needs to satisfy the documentation requirements of sections 3.5 and 3.6 of the TDG Regulations. A shipping document must therefore be created by the consignor, or the information required added to any existing document.
