Claim for Payment Forms
Two forms are required in order to make a claim for payment:

• Claim Summary Form
• Detailed Eligible Cost Form

Request for Payment by Electronic Data Interchange Form
Complete this form if you prefer to have your payment deposited directly into your bank account.

Business Performance Review
After you have completed your project, you will need to fill out this form.
