Request for Statement of Contributions

Using this online form, you can request a copy of your Canada Pension Plan Statement of Contributions. We will mail your Statement to your home address.

Please note that you can request a Statement once every 12 months. This service is available for residents of Canada only.

To use this service you will need to have the following information ready:

• your social insurance number (SIN);
• your home postal code; and
• your daytime telephone number.

Note:

• If you are currently receiving a Canada Pension Plan benefit, some estimates may not appear on your Statement at this time.
• If you live or have worked in the province of Quebec, you may need to contact the Régie des rentes du Québec (RRQ).
• It is an offence to misrepresent your identity online. In such an event, Social Development Canada (SDC) will take the appropriate action. When using this service, you agree that the information you submit to SDC is your own.
