Four Neighbourhoods Community Health Centre Four Neighbourhoods Community Health Centre is a community-based health organization staffed by an interdisciplinary team offering primary health care programs and services to individuals, families and groups.
The primary health care team provides health care services and numerous health promotion, illness prevention and health education programs. The Primary Health Care Team is the first level of contact for individuals, the family and community. Our team includes nurses, physicians, a nutritionist, a community developer, a program facilitator and administrative support staff all helping people achieve their health goals.
Community participation is valued and we have numerous volunteers assisting our programs. A Community Advisory Council and a Programming Committee also help to ensure community input.
Programs run by, or at FNCHC:
- Caregiver & Tot Program - provides parents, care givers and children with a support network.
- Breastfeeding Support Program - assists women to make breastfeeding a positive experience.
- Mommy/Daddy I Don't Feel Well - increases parent's knowledge and confidence in caring for a sick or hurt child.
- Seasonal Celebrations for all ages - annual Canada Day Celebration, Hallowe'en Party, Crafts At Confederation Court Mall, March Break Activities and Summer Camps.
- Trailblazers Walking Club - helping people 50 years and older to get to know their neighbours and keep fit at the same time.
- Youth Volunteers - partners with Birchwood & Stonepark Intermediate Schools to provide students with volunteer experience during school hours.
- Roots of Empathy - a "classroom parenting program" involving a partnering relationship between a new baby, the baby's parents, a classroom of students and a Roots of Empathy Facilitator. Children learn about baby feelings.
- Infant/Child CPR - In partnership with the Heart and Stroke Foundation of PEI and Public Health Nursing.
- "Heart to Heart Program" - offers education and support to persons with heart disease.
- Parenting Information Sessions - topics presented by various professionals on limit setting, choices and consequences and whatever topics parents suggest.
- Yoga Classes - Kripula Yoga classes.
- Information Sessions - support for families with teens in various ways, including Active Parenting of Teen sessions, topics around youth & depression, STD's, self-esteem building, sibling rivalry, eating disorders. Other information sessions include Cholesterol Reduction Classes, Understanding Menopause, and Medication Safety.
- Foot Care - clinics to keep feet in good shape so you can stay healthy & active.
- Community Groups - FNCHC provides a meeting space for many community groups like Prostate Cancer Support Group, Friends Supporting Friends, La Leche League, Changeways, TOPS.
