Q.1. Why do I need this guide?

A.1. This guide was developed to help you provide the best possible service to your disability patients. It provides an overview of the Canada Pension Plan disability program; it clarifies the role and responsibilities of the physician in helping a patient applying for or receiving Canada Pension Plan disability benefits; and it specifies what information you should provide to allow Human Resources Development Canada staff to make the best possible decision on your patient's application, without delay.

Q.2. What if I have further questions?

A.2. We would be pleased to help you. You can call us at 1 800 277-9914 and speak with one of our service delivery agents, or you can search this Web site for further clarification of our programs.

Q.3. The doctor next door to me did not receive a copy of the guide. Why not?

A.3. The guide was developed and mailed to general practitioners and family physicians, because they are usually the ones that complete the medical reports on behalf of their patients applying for or receiving the Canada Pension Plan disability benefit.

Physicians who did not receive a copy of the guide can call 1 800 277-9914 to request a copy, or they can visit our Web site at:

Q.4. I have used all the work sheets in Appendix 1 that you provided in the guide. How do I get more copies?
