Welcome. System Safety traces its roots to the early 1980s. Mr. Justice Dubin, in his commission of
inquiry into Aviation Safety, recommended that TC develop procedures in the
application of safety management concepts. He also recommended developing
proactive strategies for identifying and addressing aviation safety concerns
before the accident happens.
Similarly the Moshansky commission recommended a proactive approach to aviation safety
through a "Company Aviation Safety Management Program".
