How to Download Forms

What You will Need to Use Forms on this Site:

The forms on this site have been created in Portable Document Format (PDF) and will function on many different platforms including Windows, Mac, Unix, and Linux.

In order to open and print these forms, you require the free Adobe Acrobat Reader in a recent version on your computer, either:

If you do not have Adobe Acrobat Reader in a recent version on your computer, you may obtain your free copy by selecting the Download link below. Please note that you will now be on the Adobe Systems web site and will have to close that window to return to the departmental Forms Site.

Opening a Form:

If you already have Adobe Acrobat Reader in a recent version on your computer, you need only select the desired form number.

As soon as you make your selection, the Form Profile page is displayed on the screen with pertinent information about the form. This page is intended to assist you in determining whether this is the form you require prior to downloading. It is comprised of the following:

Details section:
This section provides basic information on the form itself as well as links to program information and information on returning the form. Forms section:
This section provides format, file name and file size information on the form: To open a form, simply select the file name. You can then print it and fill it out as you would any paper form.

Note: Some forms can be filled out on screen. You will receive a prompt indicating when a form is fillable on screen. However, once completed, the information in the completed form CANNOT BE SAVED.
