How to Download Forms
What You will Need to Use Forms on this Site:
The forms on this site have been created in Portable Document Format (PDF) and will function on many different platforms including Windows, Mac, Unix, and Linux.
In order to open and print these forms, you require the free Adobe Acrobat Reader in a recent version on your computer, either:
- Version 5.0 or greater; or
- Version 7.0 or greater (required where a screen reader is used to open and print alternate format forms designed for persons with visual impairments).
If you do not have Adobe Acrobat Reader in a recent version on your computer, you may obtain your free copy by selecting the Download link below. Please note that you will now be on the Adobe Systems web site and will have to close that window to return to the departmental Forms Site.
Download: Adobe Acrobat Reader- If you are not sure whether Adobe Acrobat Reader is already installed on your computer, simply try to open a form. If the form does not open, you require the free Reader as explained above.
- If you have the free Reader and the form still does not open, the version that you have on your computer may be outdated and the Download link will ensure that you have the latest version.
Opening a Form:
If you already have Adobe Acrobat Reader in a recent version on your computer, you need only select the desired form number.
As soon as you make your selection, the Form Profile page is displayed on the screen with pertinent information about the form. This page is intended to assist you in determining whether this is the form you require prior to downloading. It is comprised of the following:
This section provides basic information on the form itself as well as links to program information and information on returning the form.
- Number: Unique number assigned to identify the form.
- Title: Official title of the form.
- Purpose: Brief description of how form is to be used.
- Language: Language in which the form is written. This will be indicated by either E - English, F - French, B - Both French and English, or O - Other Languages (International Forms).
- Paper Size: Size of the printed form, normally 8 ½" x 11", or 8 ½" x 14". Note that some printers require manual feed of 8 ½" x 14" paper.
- Returning the Form: Details for submitting the completed form to the responsible department.
- Important Information: Details about the departmental program or service related to the form.
This section provides format, file name and file size information on the form:
- Format: Indicates that the form was created in Portable Document Format (PDF) and which version of the Adobe Acrobat Reader is required to open the form:
- PDF5 = version 5.0 or greater; or
- PDF7 = version 7.0 or greater (for alternate format forms as explained above).
- File Name: Name of the form file (e.g., ins3925b.pdf). The file name is always presented in the following order: program or service identifier (e.g., ins), form number (e.g., 3925), and language (b = bilingual) followed by the extension .pdf. The file name contains a link which when selected opens the form.
- File Size: Size in kilobytes (KB). The file size is used to provide an indication of how long the form may take to download.
Note: Some forms can be filled out on screen. You will receive a prompt indicating when a form is fillable on screen. However, once completed, the information in the completed form CANNOT BE SAVED.
Saving a Blank Form for Future Use:
This site has been designed for use by the widest possible audience. However, if you experience difficulty downloading a form, the following instructions may be helpful:- Using the right mouse button, select the desired form number.
- From the menu that appears, select Save Target As (wording could vary slightly from browser to browser).
- Save the blank form file in a directory. The blank form is now saved on your computer. You will now be able to open the form file in Adobe Acrobat Reader.
