Proof of Birth

Acceptable proof of birth can be a photocopy of any of the following documents:

• a birth or baptismal certificate;
• a birth registration;
• the hospital record of birth;
• the record of the attending physician at the birth;
• the record of the attending nurse or midwife at the birth;
• a cradle roll or other church record;
• a Record of Landing issued by Citizenship and Immigration Canada;
• a Notice of Decision issued by Citizenship and Immigration Canada;
• a valid passport; or
• a citizenship certificate.
