How to complete the Change mailing instructions request form

The first four fields are common throughout the requests.

Name of Business
To avoid processing delays, enter the name exactly as registered under the Business Number.

Business Number (BN)
Requests cannot be processed without a valid 15-character BN.

Name of Authorized Requestor
If the requestor is not an authorized person for the BN, a request cannot be processed.

Requestor's Telephone Number
Include the area code. If you are outside North America, also include the country and/or city code. Use only numbers; do not use punctuation marks such as brackets or dashes. Only one number can be entered.

The fields below are specific to this type of request.

Instruction Request

To inhibit receipt of any of the items listed, select Set from the Action field drop-down list. The start date will default to tomorrow's date if no date is entered. If the end date is indefinite for "SET" requests, enter "9999 12 31".

To commence receipt of items for which you had previously set an inhibit, select Delete from the drop-down list in the Action field for each item. Only the start date is required for "DELETE" requests. The start date will default to tomorrow's date if no date is entered.

Start Date

Enter the date the instruction should start using year/month/day format. For example, for December 31, 2005, enter "2005 12 31". The start date will default to tomorrow's date unless a date is entered. The start date may be a future date, but cannot be prior to tomorrow's date.

End Date

Enter the date for the instruction to expire using year/month/day format. This date must be later than the start date. If you prefer to have the inhibit set indefinitely, enter "9999 12 31" in the end date field. If you are expiring an inhibit previously set, leave the end date blank as only the start date is required for "delete" requests.
