How to complete the Copies of Notices and Statements request form

The first four fields are common throughout the requests.

Name of Business
To avoid processing delays, enter the name exactly as registered under the Business Number.

Business Number (BN)
Requests cannot be processed without a valid 15-character BN.

Name of Authorized Requestor
If the requestor is not an authorized person for the BN, a request cannot be processed.

Requestor's Telephone Number
Include the area code. If you are outside North America, also include the country and/or city code. Use only numbers; do not use punctuation marks such as brackets or dashes. Only one number can be entered.

The fields below are specific to this type of request.

Item Requested

Indicate the item(s) required by completing the applicable "Period Required" field to the right of the item(s). You may request only one item or one of each of the items listed.

Period Required

For a copy of a Notice, enter the reporting period end date in the Period Required field in year/month/day format. For Statements, enter the year and month of issuance. For example, for a copy of a Notice of Assessment or a Notice of Reassessment from your account, you must enter the applicable reporting period end in year/month/day format. If you enter "2006 06 30-Notice of Assessment" you are requesting a copy of your Notice of Assessment for the reporting period ending June 30, 2006.

Note
A reporting period end, reporting period, filing year, period covered and a taxation year end are terms that are used interchangeably.

For a copy of a Statement of Arrears or a Statement of Interim Payments from your account, enter only the year and month of issuance in year/month format. The day is not required. For example, if you enter "2006 07-Statement of Arrears" you are requesting a copy of your Statement of Arrears for July 2006.

Note
Copies are only available for original Notices issued after September 30, 2000, on a corporation account.
