How to complete the forms

There are several fields that are common to all request forms. The following four identification related fields must be completed for each.

Name of Business
To avoid processing delays, enter the name exactly as registered under the Business Number, including punctuation. Do not purposely abbreviate (For example, "Inc" for "Incorporated"). For sole proprietors, the name will be your legal name only, without a trade/operating name. For partnerships, it may be the name of the partnership or legal names of the partners. For corporations, enter the incorporating name or currently registered name of the corporation, if changed. This required information can be obtained from correspondence that we have previously issued to your account.

Business Number (BN)
Your Business Number is comprised of 15 characters. Nine digits to identify the business and two letters and four digits to identify each account your business may have (For example, 123456789 is the registration number and RP0002 is the account identifier). In this example the two letters, RP, identify a payroll account and 0002 identifies the account number, in this case a second payroll account). Please ensure you enter the 15-character Business Number to which your Online Request relates.

Name of Authorized Requestor
An authorized person can be an owner, partner, director, or an individual involved in the day to day activities of your business that you have previously named on your business account to have access to your confidential information for all or specific program accounts under your Business Number. It may also be a third party representative (for example: accountant, external bookkeeper, or lawyer for which you have previously forwarded a letter of consent or a completed RC59 Business Consent Form). Please enter your first and last name as registered under the Business Number.

Requests will not be processed unless the authorized requestor's name appears on the Business Number account. For information on authorizing individuals on your business account you may contact Business Enquiries at 1-800-959-5525.

Requestor's Telephone Number
Enter only numbers. Do not enter letters or punctuation marks such as brackets or dashes. Include the area code. If you are outside North America, also include the country and/or city code. A minimum of 10 characters is required for numbers within Canada or the United States. A maximum of 20 characters is allowed for international numbers. You can only enter one telephone number.
