You can download and print copies of our forms and publications for yourself, or you can order printed copies in one of the following ways:

Online
Online order form will allow you to choose which forms and publications you want to order and will be delivered to your address.

By telephone
To order by phone from anywhere in Canada and the United States, call 1-800-959-2221 from Monday to Friday 8:15 a.m. to 5:00 p.m. (local time). If you're calling from outside Canada and the United States, contact our International Tax Services Office. For TTY (Teletypewriter) service, call 1-800-665-0354 (Canada only).

Ordering 500 copies or more
To order 500 copies or more of the T4 Flat, T4A Flat, NR4 Flat, T4RIF Flat, T4RSP Flat, T5 Flat, T5007 Flat, T5008 Flat, T2202A Flat, or T2202A 3-part forms you will need to use a different form. See Information and instructions to order 500 copies or more.
