Information slips (T4 and more)

Slips are prepared by your employer, payer or administrator and must be issued by the last day of February of the following calendar year to which the slips apply. The following is a list of some of the slips you may receive:

If you know that you will not receive your slip by the filing due date, attach a note to your paper tax return stating the payer's name and address, type of income involved, and what you are doing to get the slip. Use any stubs or statements you may have to calculate the income you have to report and any related deductions and credits you can claim. Attach the stubs or statements to your paper tax return. If you are filing electronically, keep all of your documents in case we ask to see them.

 

You are able to receive your tax information slips in an electronic format. To do so, you must consent to receive them electronically, either by writing or sending an e-mail to the issuer, payer, or administrator. Contact them to see if they offer this service.

You are able to receive your Old Age Security (OAS) and Canada Pension Plan (CPP) slips electronically. This secure service is found at Social Development Canada under Tax Information Slips.

Important dates

The last day of February of the following year to which the slips apply is the deadline for employers, payers, and administrators to issue information slips.
