    A list of the personnel-pay input forms that have been approved for use in the Regional Pay System (RPS).

  SPECIAL PURPOSE FORMS ACCEPTED FOR PAY PROCESSING
    The following "special purpose" form, used for authorizing deductions from an employee's pay, is accepted by the pay offices for input to the pay system. This form is normally completed by, or on behalf of, a third party for fixed-term deductions.  The form is:

    - federal income tax arrears demands.
    This form, if received by departments for onward transmission to the pay office, should be verified to ensure that the key data it contains matches the employee's Master Employee Record, and should be transmitted to the pay office under cover of a GC 44 "Transmittal Note and Receipt" -- REFER to PPIM Section 5-6.
