Section: MISCELLANEOUS INSTRUCTIONS

Subsection: TRANSMITTAL AND RECEIPT OF MISCELLANEOUS DOCUMENTS

  Summary
    To outline the policy and procedures regarding the transmittal and receipt of miscellaneous documents that are forwarded by departments or agencies to the pay office.


  MISCELLANEOUS DOCUMENTS
  DEFINITION -- The term "miscellaneous documents" in this context refers to forms other than Personnel-Pay Input documents.
    LIST -- Miscellaneous documents include, among others, the following:

| PWGSC-TPSGC 2006* Election Form for Elective Pensionable Service
|

| PWGSC-TPSGC 2577 Request for Return of Superannuation Contributions

  PWGSC-TPSGC 2019* Notice of 35 Years of Pensionable Service

  PWGSC-TPSGC 2097 Elective Service Notice
|    
| PWGSC-TPSGC 2386 Certification Notice - Pension Support System
| PWGSC-TPSGC 2480* Election Not to Count Leave Without Pay as Pensionable Service

  DSS-MAS 8432-1 Direct Deposit -- Intercept Request

    Note: An asterisk (*) denotes those forms that support a pay action that is being requested.


  TRANSMITTING MISCELLANEOUS DOCUMENTS THAT SUPPORT PAY ACTIONS
  | WHEN TRANSMITTED -- A document (form) that supports a pay action should always be sent to the pay office timed to arrive when the transaction will be processed through the Regional Pay System (RPS). Supporting documents can be faxed to the pay office.
      Originator
  | LIST the supporting forms in free form text (FFT).
  | SUPPORTING FORMS -- An asterisk (*) indicates those forms that should normally accompany the pay action to which it refers. 
  |


  TRANSMITTING MISCELLANEOUS DOCUMENTS THAT DO NOT SUPPORT PAY ACTIONS
  METHOD OF TRANSMITTAL -- A form that does NOT support a pay action, as well as those forms that are being submitted subsequent to the pay action, should be forwarded to the pay office under cover of a CGSB Standard Form 44A -- Transmittal Note and Receipt -- or any similar form that serves the same purpose.
