2386 (Certification Notice - Pension Support System) is used to certify final salary and contribution information. This information is used by the: |

Pay Office - to report the post 1989 pension adjustment amounts, 35 years paid up date, monthly installments on arrears and deficiencies and past service arrears recovery (transfer value only). |

Superannuation, Pension Transition and Client Services Sector - to verify monthly installments on
arrears, cessation of contributions for 35 years of service, to confirm
deficiency information and to confirm recovery of past service arrears (transfer
value only).

This three-ply form is available in paper or

STRUCTURE |
|

- Part 1: To be completed by the department, Crown corporation or agency - Specifies the superannuation number, the reason for completion, the header information, the final salary, the final classification and level and the authority for the final salary of the employee.

- Part 2: To be completed by the pay office - The pay
office completes the pension adjustment amounts for reciprocal transfer
agreement service, certifies the arrears deductions for elections,
reports any known deficiency information and certifies the recovery
of past service arrears (transfer value only).
