Enter your identification information

Name of business
To avoid processing delays, enter the name exactly as registered under the Business Number.

Business number (BN)
A request cannot be processed without a valid 15-character BN.

Name of authorized requestor
If the requestor is not an authorized person for the BN, a request cannot be processed.

Last name

First name

Name of authorized firm
Enter the name of your firm if you are an authorized representative making a request for a client.

Requestor's telephone number
Include the area code. If you are outside North America, also include the country and/or city code. Use only numbers; do not use spaces or punctuation marks such as brackets or dashes. For example, enter (123) 456-7890 as 1234567890.

Review the information you are submitting to make sure it is correct. If correct, select "Continue". If not correct, make the necessary changes or select "Clear form" to start over.
