PURPOSE

The purpose of this advice is to inform users of the appropriate method of
supply for the following forms:
The
Public Service Health Care Plan - Employee Application, previously numbered
PWGSC-TPSGC 2243 has now been changed to TBS-006491.
The Public Service Health Care Plan - Pensioner Application,
previously numbered PWGSC-TPSGC 2243-1 has now been changed to
TBS-006492.
The TBS-006491 is completed
by employees who wish to join the Public Service Health Care Plan or amend
their coverage under the plan.
The TBS-006492 is
completed by employees who wish to join the Public Service Health Care Plan or
amend their coverage just before retirement.
