	1.	PURPOSE
	1.1	The purpose of this advice is to inform users of the appropriate method of supply for the following revised form: PWGSC-TPSGC 2064, War Service Questionnaire. The PWGSC-TPSGC 2064 is used to gather information regarding service with the Canadian Forces during World War II.
	2.	PROCEDURES
	2.1	This form is decentralized and can be ordered from one of the printing plants, listed below, using a "Detailed Printing Order Form", CCG/GCC 3149-2A. The electronic version of the PWGSC-TPSGC 2064 is also available on the "Elf " software System.
