Registration Forms
The forms on this page can be used to
apply for a new registration, challenge a pending application for
registration, change data in the OSI registry, or deactivate an
existing entry.
You can complete the necessary forms on-line using the HTML versions,
or print out the PDF versions and submit them by mail or fax. You
may also request hard-copy versions of the forms from the Registration
Administrator (see contact information below).

forms for each Information Object you wish to register, challenge,
