Application Package for all Industry-Partnered Research Training Awards (excluding Industry Partnered Operating Fellowships and Clinician Scientists Phase 1)

For Industry-Paretnered Doctoral Research Awards, Fellowships, and Master's Awards

1. Review necessary documentation in the CIHR Partnerships Programs Guidelines and the Current Funding Opportunities.

2. Review the following PDF forms to familiarize yourself with the application requirements:

3. Read and print the following Guidelines for the Completion of the Modules:

4. Have the appropriate Sponsors reports ready:

5. Have your PIN ready.

6. Complete the Training Module - [Web Forms].

7. Complete the Common CV (Validated for CIHR) for student and supervisor(s) - http://www.commoncv.net/ [ External Site ]

8. Complete the Industrial Partnership Module - [Web Forms].

9. Complete the Innovation and Industry Program identification page - [PDF (220Kb) | Help] and place it on top of the original copy.

10. Send 3 copies by courier to:
