Operating Fellowships - Industry Partnered Full Application Package

1. Review necessary documentation in the CIHR Partnerships Programs Guidelines and the Current Funding Opportunities.

2. Print and read the following PDFs to familiarize yourself with the application requirements:

4. Have the three Sponsor's reports ready - [PDF (fillable, 58 KB) | PDF (31 KB) | Help ].

6. Complete the Training Module - [Web Forms]. Make sure to write Industry-Partnered Operating Fellowship on the top of the page.

7. Complete the Common CV (Validated for CIHR) for fellow and supervisor(s) - http://www.commoncv.net/ [ External Site ]

8. Complete the Operating Budget Module - [Web Forms].

9. Complete the Industrial Partnership Module - [Web Forms].

10. Complete the Innovation and Industry Program identification page - [ PDF (220KB) | Help ] and place it on top of the original copy.

11. Send 3 copies by courier to:
