Implementing the Population Health Approach

checkpoints for applying the population heath approach

Implementing a Population Health Approach:

  • is a cyclical process - and there's not necessarily a linear step by step way to apply the method to project work. You can start anywhere on the wheel, and go over the same territory in cycles. The roles of the partners evolve over time.
  • involves shifting resources (to a degree) into earlier intervention, prevention, and promotion
  • can include many different strategies - not necessarily one model
  • involves enrolling people into working together for health - developing collective commitment and working on systems and determinants together.
