ANNEX E

INSTRUCTIONS FOR COMPLETING THE EXPORT CERTIFICATE FOR U.S.A AND THE CONTINUATION SHEET

  1. Serial number of the export certificate.

  2. Product description, eg. boneless beef, carcass lamb, corned beef, roast beef, chicken nuggets, duck carcasses, etc. together with the Protein Fat Free (PFF) group for PFF products and Group 2 for cooked sausage, if applicable.

    Note: When the product bears an establishment number on its protective covering/immediate container and is repackaged in a shipping container with another establishment number, write "Produced at est. # ......" next to the product description.

  3. Bovine, cattle, calf, swine, porcine, pig, equine, horse, ovine, lamb, sheep, caprine, goat, and kid, are acceptable. (Not required for poultry).

  4. Self explanatory.

  5. Net weight in pounds and ounces.

  6. Information to enter in this space must be the producing establishment number, which must appear as the producing establishment number on form 9540-1 section 11, on the shipping container and where applicable on the product. This is the establishment number that will be entered in the FSIS import control system. This establishment must be approved for export to the USA.

  7. Information to enter in this space must be the establishment number of the exporting establishment and which must appear as the shipping establishment number on form 9540-1 section 10. All Canadian registered establishment numbers can be used as a shipping establishment since they are all eligible as a shipping establishment.

  8. Province code (see Chapter 11, Annex F - Introduction).

  9. Name of the exporter (may be different from the producing establishment).

  10. Address of the exporter (may be different from the producing establishment).

  11. Name of the consignee.

  12. Destination could be the port of entry or the consignee’s business address.

  13. Export stamp number that is applied on shipping containers: the number used is the serial number on the corresponding certificate. When boxes are stamped and the certificate must be replaced, the number of the certificate replaced must appear in the remark section. (See 14 when a certificate must be replaced and the shipping containers are stamped).

  14. Use this space for certificates issued in lieu of. The following statement should be used: "Certificate issued in lieu of certificate No. _______".

  15. Self explanatory.

  16. Date on which the certificate was signed and issued.

  17. Print the name of the signing officer and Veterinarian or D.V.M. Apply the seal to each copy of the certificate.

  18. The signature must be in a colour different from that of the printing.

NOTE: The unused space on the certificate must be voided by means of a diagonal line.
