Client Satisfaction Survey

Thank you for agreeing to participate in our survey. Your answers will help the Division of Aging and Seniors (DAS) to understand your expectations and preferences and to meet your health information needs better.

All information will be kept in the strictest confidence. No individual will be identified in any way in our report. Responses will be collected and reported as a group (see our privacy policy for more details).
