Who may
order a duplicate change of name certificate:
- the person whose name appears on the change of name
registration or when requested by his or her legal
representative (e.g., parent, guardian, solicitor
or legal agent)
- member of the immediate family if the person whose
name change is recorded is deceased or incapable of
making the request on his or her own behalf
- spouse of a person whose name appears on the change
of name registration
- an agent, or any other person, on the written authorization
of the person or the parents
- a person who requires the certificate for use in
a court of law, for adoption, or for settlement of
an estate
- an officer of the Crown for official purposes.

of Name Certificates
Certificate of Change of Name (21.5 cm x 35.4 cm)
- Contains former name, address, date of birth, new
name, registration number, registration date and,
if applicable, former name of spouse, spouses
date of birth, spouses new name, former name
of child (children), new name of child (children)
and childs (childrens) date of birth.
