Death Certificates
A death certificate is required:
- as legal proof of the death
- to settle an estate
- to receive entitlements for benefits such as pensions
and insurance.

- member of the immediate family or by the next of
kin
- a person who requires the certificate for use in
a court of law, for settlement of an estate or for
disinterment purposes
- an officer of the Crown for official purposes
- government departments approved by the Director
of Vital Statistics when required for legal or other
legitimate purposes.

How to order a death
certificate
- Complete an Application
for Certificate form, providing all relevant information with respect to the
application.
- The fee
must accompany the application in the form of a cheque,
money order or credit card (VISA or MasterCard). Cheques
or money orders should be made payable to Saskatchewan
Health.
- Apply in person at the Vital Statistics office,
by fax (if paying by credit card only) or by mail.

Types of death certificates
Frame Certificate (21.6 cm x 17.8 cm)
- Contains the full name of the deceased, date of
death, place of death, age, sex, marital status, registration
number and registration date.

Certified Photocopy of Death Registration (21.5 cm
x 35.4 cm)
- Contains all of the information that appears on
the original death registration excluding cause of
death.
Genealogical Photocopy of Registration (21.5 cm x 35.4
cm)
- Contains all of the information that appears on
the original death registration excluding cause of
death. The document is stamped "FOR GENEALOGY
ONLY."
