BOARD OF INQUIRY SITUATIONAL REPORT #15 FIRES AND CASUALTIES - HMCS CHICOUTIMI

1. The Board of Inquiry continued to investigate the actions and decisions surrounding repairs made to the upper lid vent in accordance with your direction (Reference B).

2. Over the past week the Board heard from five witnesses.  The individuals called included a naval architect with expertise on Victoria class seakeeping, a senior officer of the crew, and members of the repair party who worked on the submarine’s upper lid vent.

3.The crewmembers notified of the potential for adverse evidence were present, with legal counsel, for all testimony.

4. More testimony is scheduled for 6 March and will involve other Canadian submarine Commanding Officers.  It is anticipated that additional testimony will be received in the coming week.

5. The current schedule will preclude the delivery of the supplementary report on 8 March 2005.  Therefore, a request for extension to 31 March 2005 was forwarded at reference C.
