Guardianship Investigations If you are a Health Practitioner, or a Community Care Access Centre and require a decision from the OPGT under the Health Care Consent Act, contact the Treatment Decisions Unit at the applicable regional office noted below. Matters concerning property guardianship clients of the OPGT, or inquiries on other matters, may be directed to Client Services at the applicable regional office: Head Office (Greater Toronto Area)
