You may receive application forms from the Office of the Registrar General, or in-person from our Toronto office. You can pick up (only) forms for birth, marriage or death certificates from Government Information Centres located across the province.

If you are applying for a birth certificate for an individual age nine or older, guarantor information is required. The guarantor is not required to sign your application, but must give you permission to provide his or her information.

You may also complete this form online and print it. Mail or fax the completed form to the Office of the Registrar General or bring it in person to one of our counter locations. The service guarantee does not apply to applications submitted by mail, fax or in person.

Online Marriage and Death Certificate Applications 


These forms are used by individuals to request proof of registration (certificates, certified copies) of marriages or deaths occurring in Ontario. Use the Office of the Registrar General's online service to apply for marriage and death certificates any time of day or night. The form can be filled in online, printed, mailed or faxed to the Office of the Registrar General, or brought in person to one of our counter locations with the appropriate fee for processing.

The Marriage Licence Application is completed by individuals wishing to purchase a Marriage Licence to be married in Ontario. The form will only be accepted by completing the information and taking it in person to an Ontario Marriage Licence issuer. You will need to bring with you identification, such as a birth certificate (along with any change of name certificates), current passport, Record of Immigrant Landing or Canadian citizenship card, along with photo identification, for both the bride and the groom. Contact your City/Municipality Clerk's Office for the location of the Marriage Licence Issuer nearest you and any further requirements.
