Certificates, Licences and Other Cards

Credit Card Companies

Contact the financial institution or credit card company that issued your card.


Ontario Birth Certificate

If you were born in another province or country, you will need to contact that province or country in order to replace your birth certificate. Contact the Ministry of Government Services for more information.


Ontario Health Card

You will need three pieces of identification in order to replace your Ontario Health Card. For information contact your local Ministry of Health and Long-Term Care office.


Social Insurance Number

You will need a primary form of identification in order to get a replacement Social Insurance Number (SIN) card. Contact Human Resources and Social Development Canada to obtain a another card.


Ontario Driver's Licence

You will be required to provide proof of identification to pick up a replacement Ontario Driver's Licence. Go to your nearest Driver and Vehicle Licence Issuing Office or contact the Ministry of Transportation for more information.


Canadian Passport

You will need an original document proving Canadian citizenship in order to replace your passport. For more information, contact the Department of Foreign Affairs Canada.


Status Card

You will need two pieces of identification in order to replace your status card. For information contact your local Indian and Northern Affairs Canada.


Canadian Citizenship Card

You will need two pieces of identification in order to get a citizenship card. Contact Citizenship and Immigration if you require more information.


Immigration Record of Landing

Please contact Citizenship and Immigration Canada at their toll-free telephone service.


Permanent Resident Card

Refer to the Document Checklist for information on obtaining a replacement Permanent Resident Card (PR Card).


Bring Your ID Card

You will need a birth certificate, a citizenship card, a valid passport, or notarized affidavit showing name and date of birth in order to get a Bring Your ID (BYID) card. For information on this card contact the Liquor Control Board of Ontario (LCBO).


Outdoors Card

You can get an Outdoors Card without any identification. Contact the Outdoors Card Customer Service Centre for information.


OAS (Senior's Card)

You can replace your Old Age Security (OAS) Card without any identification but you will need to know your Social Insurance Number (SIN).


Firearms Licence / Firearms Acquisition Certificate

You can replace your Firearms Licence without any identification. Contact the Canadian Firearms Centre for more information.


Pleasure Craft Operator Card

To replace your Pleasure Craft Operator Card, contact the private sector organization that issued the card.
