Company Forms

The ministry is committed to providing clients with greater access to Companies and Personal Property Security Branch (CPPSB) forms. We are always looking for new ways to provide more convenient service. New forms will be provided as they are released, so please check our site regularly. For instructions on how to access and fill out these forms, see Instructions below.

Business Corporations Act (BCA)

Forms for business corporations

Business Names Act

Forms for registering, renewing, amending or canceling a name registration for a sole proprietorship, general partnership, corporation, Ontario limited liability partnership, extra-provincial limited liability partnership or extra-provincial limited liability company.

Corporations Information Act (CIA)

Instructions

1) The forms can either be completed and printed online (min. Adobe 4.0 required), or a blank copy of the form can be printed for completion at a later date.

2) The forms must be signed (original signatures on both copies) and submitted in duplicate to the Companies and Personal Property Security Branch by mail or in person at the Public Office.

3) Forms transmitted electronically to Companies and Personal Property Security Branch will not be accepted for filing.

Note: To avoid using outdated versions, we recommend that you do not download forms to your hard drive.

Viewing the forms
The forms are in Portable Document Format (PDF). To view PDF files, you need to have Adobe Acrobat® Reader version 4.0, or later, installed on your computer. You can download this free software from the Adobe Web site. Note that earlier versions of Acrobat® will not work properly with these files and there are some issues with Version 3 that may cause printing problems.

Filling in the forms

  1. The form will open with the pointer in the start position. Begin typing your information.
  2. Use the TAB key on your keyboard to move to the next box. You can use SHIFT/TAB to move back.
  3. Click your mouse to fill-in "checkboxes".
  4. You can use the CLEAR FORM button on the form if you need to start over.

Printing your forms
Acrobat® Reader will not allow you to electronically save the information you have entered into your form. You must print a copy of the completed form before closing the file.

  1. After completing the form on your screen, look at the status bar below the form image. It will show you the correct paper size to use when printing. Note that some forms may be more than one page, or may be in "landscape" format.
  2. The Companies and Personal Property Security Branch requires two copies of all forms submitted under the Business Corporations Act, the Corporations Act and the Extra-Provincial Corporations Act. Sign your name on both copies where indicated.
  3. Mail both copies of your completed information to the return address shown on the form (include appropriate fee where required) or visit the CPPSB office in person.

If the printed form is being cropped on the bottom of your page, there are several solutions:

  • On the Acrobat® Reader menu, select:
    FILE > PRINT MENU; select FIT TO PAGE
  • Reduce your printer margin settings.
  • Reduce the resolution setting of your printer from 600 dpi to 300 dpi.
  • Increase the RAM memory of your printer.
  • Set the dithering option to "Fine" or "Coarse"
  • Set the graphics option to "Raster" mode. (Not available for all printer drivers).
