To have your address changed, you can :

  • Complete the on-line form
  • Phone, write or go to your regional branch of the CSST
The procedure is the same for industrial accidents and occupational diseases. If you are receiving compensation for an industrial accident and for an occupational disease, be sure to say so when you call.

Note
If you received compensation during part of the year and you’ve since moved, you must notify the CSST of your new address in order to obtain the RL-5 slip to be attached to your income tax return.
