Notify the CSST’s Direction de l’indemnisation des victimes d’actes criminels (IVAC) by telephone, mail or e-mail. You must specify your file number.

The procedure is the same for compensation paid to victims of criminal acts and compensation paid to persons injured while performing an act of good citizenship.

Note
If you received compensation during part of the year and you’ve since moved, you must notify IVAC of your new address in order to obtain the RL-5 slip to be attached to your income tax return.
