Call the Régie de l’assurance maladie, or go to its Québec or Montréal office. Be sure to have your health insurance card with you. If you do not have it on hand, give your social insurance number, name and date of birth.

You must notify the Régie of any change of address in Québec. It’s the only way to ensure that you receive a renewal notice before your card expires.

At the same time, the Régie will make the change for the other people living with you. Remember to give their names.

Note
Under the Health Insurance Act, the Régie is required to transmit, to the Directeur général des élections du Québec, all the change of address notices it receives from persons on Québec’s permanent list of electors so that the list can be updated.
