Registration certificate
Complete an Application for Registration (form LM-1-V), and file it with Revenu Québec along with a document showing the address of each establishment you operate or cause to be operated by another party.
Your registration certificate must be prominently posted at your main place of business; a copy must be posted at each of your other establishments.
