Application for a Reduction in Source Deductions of Income Tax

This form is for use by an individual who wishes to request that Revenu Québec authorize his or her employer or payer to reduce source deductions of income tax because the individual believes they are too high for one of the following reasons:

• the individual is entitled to deductions or tax credits that the employer or a payer cannot take into account without the authorization of Revenu Québec
• the individual is not a resident of Québec and is not subject to the Taxation Act.
