Application Forms

Please review the Introduction home page and the Application Process Flow Sheet to gain an understanding of how the application process works and the application forms are to be used.

As the process covers large and small projects, the pre-application meeting is essential to setting the scope of the application process requirements. The Introduction also leads applicants through the guidance documents on Completing an Application Form, Consultation, and Technical Assessment.

The Environmental Management Act Waste Discharge Authorization application form consists of nine parts. The type of application being submitted determines the parts each applicant is required to fill out. A brief explanation is provided in each section below.

Part 1. General Applicant Details

This form must be completed by all applicants.

Part 2(a). Environmental Protection Notice:
New Application

This form must be completed by applicants who are applying for a new permit or approval.

Part 2(b). Environmental Protection Notice: Amendment

This form must be completed by applicants who are applying for an amendment to an existing permit or approval.

Part 3. Discharge Details

A separate form must be completed by all applicants for each point of discharge or storage site.

Part 4. Receiving Environment

This form must be completed by applicants who do not provide a technical assessment report.

Part 6. Consultation Report

Part 7. Technical Assessment Report

The Regional Manager will indicate to the applicant during the pre-application meeting whether or not a technical assessment will be required. Applicants who must complete a technical assessment should refer to this report.

Part 8. Application Fee

Applicants must determine and submit the appropriate application fee. For information, go to the Guidance — Applications website (page 7).

Part 9. Agent Authorization / Applicant Signature

Applicants who choose to have an agent work directly with the ministry on their behalf must complete this form.
