Application Forms

Reminder: Please review the Eligibility section for information on the licences you may qualify for.

  • All forms are available in PDF format. Click on the interactive form from the list below that you want to view.
  • To view the documents in PDF format you will need to have the free Adobe Reader installed on you computer. Click here if you do not have Adobe Acrobat Reader.
  • When filling out the forms you can:
    • complete the forms online by pointing your mouse to the area that you want to fill in and click. You may move forward through the form with the TAB key, or backwards with the SHIFT + TAB key;
    • print the forms by clicking on the print icon in the Adobe menu bar and fill out any remaining sections with pen;
    • save the blank forms to your local computer by clicking on the save icon in the Adobe menu bar; and
    • submit the forms to SLGA as per the instructions provided within each form.

Bingo Addendum Request.pdf
This form is to be used by a bingo licensee to make changes to any of the information that appears on their bingo licence.

Bingo Association Addendum Request.pdf
This form is to be used by an association bingo hall to make changes to their scheduled events.

Breakopen Addendum Request.pdf
This form is to be used by a breakopen licensee to make changes to any of the information that appears on their breakopen licence.

Breakopen Application.pdf
Use this application form to apply for a licence to sell breakopen tickets if you are a charitable or religious organization.

Breakopen Unsold Inventory Report.pdf
This form must be filled out by any charitable or religious organization that holds a breakopen licence. It is to be submitted at the end of their breakopen licence (or if renewing, submitted with their renewal application).

Class A Breakopen Monthly Report.pdf
To be used by a Class A breakopen licence holder, who pools breakopen revenues in an association bingo hall. It must be filled out monthly and submitted to SLGA.

Class A Expense Details.pdf
To be used by a Class A licence holder that is an association pooling hall. It is to be filled out monthly and submitted to SLGA.

Class A Pooled Breakopen Disbursement.pdf
To be used by a Class A breakopen licence holder, who pools breakopen revenues in an association bingo hall. It must be filled out and submitted to SLGA.

Class A Pooling Close Out.pdf
To be used by a Class A licence holder in an association pooling hall. It must be filled out after each bingo event, and at the end of the month submitted to SLGA along with the corresponding pooling event log within five (5) days of the end of the calendar month.

Class A Pooling Event Log.pdf
To be used by a Class A licence holder in an association pooling hall. It must be filled out at the end of each month of the licence and submitted to SLGA, along with the corresponding months’ close-out forms, within five (5) days of the end of the calendar month.

Class A Regular Close Out.pdf
To be used by a Class A licence holder in an association hall. It must be filled out after each bingo event, and at the end of the month submitted to SLGA along with the corresponding event log within five (5) days of the end of the calendar month.

Class A Regular Event Log.pdf
To be used by a Class A licence holder in an association hall. It must be filled out at the end of each month of the licence and submitted to the SLGA, along with the corresponding months’ close-out forms, within five (5) days of the end of the calendar month.

Class B Bingo Application.pdf
Use this application form to apply for bingo and breakopen licences if you are a charitable or religious organization who has received approval from an association bingo hall to run bingo events within that hall.

Class C Bingo Application.pdf
Use this application form to apply for bingo and breakopen licences if you are a charitable or religious organization who will be running no more than three bingo events a week in a community bingo hall and where the prize value per event exceeds $1,000.

Class C Close Out Summary.pdf
To be used by a Class C bingo licence holder after each bingo event.

Class C Monthly Bingo Event Log.pdf
To be used by a Class C bingo licence holder every month and submitted to SLGA within five (5) days of the end of the calendar month; it provides a summary of all close-outs for the month.

Class C Restricted Bingo Application.pdf
Use this application form to apply for bingo and breakopen licences if you are a charitable or religious organization who will be running only one bingo event per week in a community bingo hall and where the prize value per event is less than $1,000.

Class C Restricted Financial Report.pdf
To be used by a Class CR bingo licence holder once for the first six (6) months of the bingo licence and then again for the remainder of the licence and submitted to SLGA within twenty (20) days of the end of the reporting period.

Regular and Large Raffle Application.pdf
Use this application form to apply for a raffle licence if you are a charitable or religious organization and the total retail prize value for all draws will be greater than $1,000.

Regular and Large Raffle Financial Report.pdf
For raffle licences that have a retail prize value greater than $1,000. It must be filled out after the final draw has been made and submitted to SLGA within twenty (20) days of the final draw date.

Monte Carlo Application.pdf
Application form to obtain a Monte Carlo licence from SLGA.

Municipal Raffle Lottery Financial Report.pdf
For raffle licences that have a retail prize value less than $1000. It must be filled out after the final draw has been made and submitted to SLGA within (20) days of the final draw date.

Public Amusement and Municipal Raffle Application.pdf
Use this application form to apply for a raffle licence if you are a non-profit organization with a prize value of $500.00 or less or if you are a charitable or religious organization and the total retail prize value for all draws will be less than $1,000.

Public Amusement Lottery Raffle Financial Report.pdf
For raffle licences that have a retail prize value less than $500. It must be filled out after the final draw has been made and submitted to SLGA within (20) days of the final draw date.

Raffle Addendum Request.pdf
This form is to be used by a raffle licensee to make changes to any of the information that appears on the raffle licence.

Texas Hold em Application.pdf
Application form to obtain a Texas Hold 'em licence from SLGA.

Trust Account Reconciliation Report.pdf
To be used by Class A licence holders, both pooling and non-pooling halls. It must be filled out at the end of each month of the licence and submitted to SLGA within five (5) days of the end of the calendar month.
