Special Support Application Forms
We have automated the renewal process with Canada Revenue Agency (CRA), so that you will not be required to re-apply every year. This automatic process is more efficient, reduces duplication, and ensures continuous coverage. If you complete the one time Special Support - CRA Application/Consent Form ("Side A"), it will authorize the Drug Plan to obtain your income tax information from the Canada Revenue Agency, so you do not have to apply each year. If you decide you do not want the automatic renewal process, the Special Support - Annual Application ("Side B") allows you to complete a renewal form and submit it with your income information each year.
