When filling out the forms you can:
complete the forms online by pointing your mouse to the area that you want to fill in and click. You may move forward through the form with the TAB key, or backwards with the SHIFT + TAB key;

save the blank forms to your local computer by clicking on the save icon in the Adobe menu bar; and
submit the forms to SLGA as per the instructions provided within each form.

Absentee Trainer.pdf This form names a qualified replacement when a trainer will be away from his training responsibilities for any period of time.
Additional Information Form.pdf This form is to be used when a licensee answers "yes" to any of the questions on the Application Form regarding previous horse racing licensing and criminal history information, with the exception of question 9.
Authorized Agent.pdf This form is used to designate one person to act on behalf of a partnership/stable name or another licensee in all matters pertaining to horse racing.
Bill Of Sale.pdf This form must be completed when one party sells a horse to another party.
Change Of Trainer.pdf This form must be completed when an owner ceases to use the services of one trainer for another trainer.
Licence Application - Supplementary.pdf This form is used when a licensee with a valid SLGA Horse Racing licence changes or adds a category to his licence. Also used when a lost licence must be replaced.
