Who needs this form? All students applying for Canada-Saskatchewan Integrated Student Loans. To protect your privacy, we will not release information to a third party such as your parents or spouse unless the Consent to Release Information Form is completed and submitted to both Student Financial Assistance and the appropriate division of the National Student Loans Service Centre. This form does not substitute for a Power of Attorney**.
Study Grant Applications/supporting documents:
Who needs this form? Students with a permanent disability that require education related services and/or equipment. The Calendar for Support Services is submitted to keep track of hours of tutor, scribe, reader, etc.
Who needs this form? Female students who are enrolled in full-time PH.D. studies in selected fields of study.
Who needs this form? Recipients of any of the above Canada-Saskatchewan Study Grants may be required to confirm their enrollment.
Reassessment Forms:
Who needs these forms? Students who require a reassessment due to medical reasons, reduced parental income, non-liquidatable assets, change to family size or marital status.
