*	Fingerprints are required when an individual declares a criminal offence, if out-of-country checks are required, or if requested by CIISD. Refer to Chapter 2 of the Industrial Security Manual, revised October, 2002 for Personnel Security Screening Instructions.

**	CIISD will issue this form when a security screening is granted. Parts C & D are to be completed and a copy returned to CIISD once the individual has been briefed. This form must also be submitted when an individual no longer requires a security screening or leaves your organization. Refer to Chapter 2 of the ISM.

	Electronic submissions A hard copy of the electronic submission bearing original signatures and the Fingerprint Form (if required - refer to * above) is to be submitted to CIISD.
