Request for shipment by specification I-15

The form was designed for users to complete electronically and send via email.

What to do

  1. Open the form in Microsoft Word
  2. Enter information in each form field. Use Tab or arrow keys to move between fields.
  3. On the File menu, select Save As
  4. In the File name box, name the completed form and save.
  5. Attach the completed form to an email.
  6. Send to the Regional Director located at the shipping location.

Adobe PDF format

If you are unable to use the electronic Word form, print the PDF format, complete it and fax to the Regional Director located at the shipping location.
