What to do

1. Open the form in Microsoft Word
2. Enter information in each form field. Use Tab or arrow keys to move between fields.
3. On the File menu, select Save As
4. In the File name box, name the completed form and save.
5. Print the grain treatment report.
6. Mail along with all post treatment samples to:
   Winnipeg Service Center
   Industry Services
   1054 Pembina Highway
   Winnipeg  MB R3T 1Z8
