All nomination material should be provided on 8½" x 11" paper. Material should be single-sided to facilitate reproduction for judging. Do not use staples or other binding. Material may be paper clipped.

Please enclose the following material:

  1. Statement of nomination:
    • Describe in approximately 250 words the reasons to consider the Nominee. This statement should include a description of activities the candidate has undertaken with respect to promoting the equality of women.

    • The statement should contain descriptions of volunteer or paid work and the names of organizations. Describe the level of the candidate's responsibilities within these organizations or the community.

    • Describe the innovative achievement(s), lobbying or advocacy efforts the candidate has undertaken with respect to improving equality for women and girls in Canadian society. Provide detailed specific examples of the results of the candidate contributions, including solutions to specific problems, outstanding achievements or laws that were changed as a result of the candidate's work.

  2. Candidate's résumé or curriculum vitae:
    • Provide a separate outline of the candidate's background, career, and current work. Include volunteer work, education, awards or other notable achievements.

  3. Letters of support:
    • Provide a minimum of two (2), and a maximum of four (4), single-page letters written in direct support of the nomination.
