The washer in the tether anchorage kit does not comply with the required salt spray tests specified in the applicable
Canadian Motor Vehicle Safety Standard (CMVSS) 213.5. The washer could potentially rust and weaken the part resulting in a failure during a collision. |

During routine compliance inspection, the company identified the problem and immediately initiated an investigation to confirm their findings. A safety campaign was initiated to replace the defective washer. It is understood that all the owners can be identified and contacted because the units have a unique identification number which can be cross-referenced to individual customers within the disabled community. |
