Access to Information Act

This request may be used by Canadian citizens, permanent residents or other individuals or corporations present in Canada to request access to information from the Department of Human Resources and Skills Development Canada (HRSDC) or the Department of Social Development Canada (SDC) under the Access to Information Act.



Step 1

To apply for information under the Access to Information Act,  complete this form, describing the information being sought and provide any relevant details necessary to help the Department find it. If you require a description of program records held by the Department, please refer to:

Step 2

Submit the access request to the Office of the department's Privacy Coordinator listed at the bottom of this page. Enclose a $5.00 money order or cheque payable to the Receiver General of Canada, or telephone your Mastercard or VISA payment directly to the Privacy Coordinator. You will receive an acknowledgement which will provide you with a number for tracking progress on your request. Depending upon the type or amount of information being sought, you may be asked to authorize further charges.

Step 3

When you receive an answer to your request, review the information to determine whether you wish to make a further request under the Act. You also have the right to complain to the Access to Information Commissioner should you believe that you have been denied any of your rights under the Act.

Affirmation and Payment

Under the Access to Information Act, requests for access to information can be made by a Canadian citizen, permanent resident or another individual present in Canada, or a corporation present in Canada. Please indicate your situation:

Your initial charge of $5.00 to activate this request can be made through cheque, money order, or credit card. Payment is to be made payable to the Receiver General of Canada. Please read the available options below and select your method of payment:


Cheque or money order: Payment should be sent to: Office of the HRSDC/SDC Access to Information Coordinator, Human Resources Development Canada, Place du Portage Phase IV, 1st floor, Hull, Quebec, K1A 0J9. Your request will be initiated immediately; however, if payment is not received within 30 days, this request will be cancelled.
Credit card - by telephone: Payment can be made by Mastercard or VISA over the telephone by contacting the department's Access to Information Coordinator, at: 1 (819) 994-0416

Your request will be initiated immediately; however, if payment is not received within 7 days, this request will be cancelled.

Please note that, depending upon the type or amount of information being sought, you may be requested to authorize further charges.




Your completed request must be submitted to the HRSDC/SDC Access to Information Coordinator in order to be activated. Once your request and payment are confirmed, you will be provided with a file number - to use in further contacts and tracking progress on this request.
